PeaceHealth is seeking a talented Licensed Clinical Social Worker to support the primary care practice at Barger Clinic as a Behavioral Health Consultant, in a Behaviorist role.

Behavioral health care is an integral component of the Patient Centered Primary Care Home (PCPCH) which focuses on the psychological and social determinants that affect overall health. This key role supports the clinics and providers with managing care management cases, chronic medical conditions, behavioral issues, anxiety, stress, financial challenges, housing issues, medication issues, disability support, patient communication, and assessments for behavioral health referrals.

Qualifications for the Behaviorist role include:

  • Masters’ in Social Work (MSW)
  • Licensed Clinical Social Worker licensure in Oregon
