RN Complex Care Manager - Primary Care

PeaceHealth is seeking a RN Complex Care Manager - Primary Care for a 1.0 full time equivalent Exempt position. This position will be working Day shift with shift durations of 8 Hours.

This position will work at our RiverBend Primary Care Clinic overseeing and evaluating coordination of integrated population management across selected populations to improve clinical and financial outcomes. Develops care management programs addressing a wide scope of health care needs and conditions.

We are seeking a relationship-centered nurse with strong leadership skills, strong patient advocacy skills, and strong teamwork behaviors. Bring your knowledge of health coaching techniques and ability to manage multiple patient encounters through organizational skills and proficiency in applying highly technical principles, concepts, and techniques.



  • Minimum three years of clinical nursing experience in an ambulatory or clinic setting required.
  • Current Oregon State Registered Nurse Licensure. 
  • Valid Oregon drivers’ license.  Responsibilities may include home visits as needed and/or travel to and from clinic sites at multiple locations.
  • Care Management or Care Coordination experience preferred.
  • Utilization management/quality management experience preferred.



Oregon West Network:

  • Educational experience:  deliver patient education and teaching including medication injections and medical equipment training (Glucometer).
  • Lengthy computer work on reports and clinical chart may be required.
  • Valid Oregon drivers’ license.  Responsibilities may include home visits as needed and/or travel to and from clinic sites at multiple locations.
  • For grant projects:  may be required to manage all metric reporting on clinical data and outcomes for the patient population reporting under the grant.



  • Work requires normal amounts of physical effort up to 33% of the time. 
  • Ability to lift objects weighing 30 lbs. or less.   
  • Work requires frequent exposure to minor cold, heat, poor ventilation or sharp instruments. Reasonably anticipated exposure to blood and body fluids once per month or more.  
  • Job duties frequently require intense concentration and attention to detail (34-65% of work time).
