RN - MDS Coordinator - Long Term Care

PeaceHealth is seeking a RN - MDS Coordinator - Long Term Care for a 1.0 full time equivalent Exempt position. This position will be working Day shift with shift durations of 8 Hours.

 Accountable for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process in this unit.  Promotes the resident’s quality of care and services through assessment and analysis of quality indicators and development of performance improvements.  Assures appropriate reimbursement through accurate and timely compliance with federal/state requirements.



  1. Plans, organizes develops and coordinates the completion of documentation and the input of patient assessment data as required by federal and/or state regulations for reimbursement. Assures accurate transmission of data in accordance with regulations and maintains a compliance log of transmissions.
  2. Leads the development, implementation, outcome and evaluation of a plan of care for each resident in the unit.  Leads interdisciplinary teams to ensure resident issues/needs are identified and care plan and goals are established and measured and adapted to ensure goals are met.    
  3. Ensures accuracy of care plan and endorses and certifies by their signature on the care plan. 
  4. Maintains updates and revises required documentation according to patient changes in condition, as regulated by federal and/or state guidelines. Effectively communicates and coordinates with members of the interdisciplinary team regarding appropriate functional/care level and transition to another level. Advises patients of rights regarding information transmitted.
  5. Performs audits related to the reimbursement documentation and the quality management process. Reviews audit results and presents summary information and recommendations for management utilization and review.
    • Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans.
  6. Ensures compliance from a risk management/regulatory perspective through early identification of clinical events or trends. Provides data management and summary information regarding clinical decisions for benchmarking and comparative analysis.
  7. Maintains education and updated knowledge of federal and state regulations in order to assure appropriate reimbursement. Provides education and guidance to the interdisciplinary team regarding regulations, reimbursement system, and associated documentation to assure maximum reimbursement levels.
  8. Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.
  9. Maintain and update written policies and procedures that govern the development, use, and implementation, of the resident assessment (MDS) and care plan, in collaboration with the unit Director and / or Charge Nurse.




  • Registered Nurse (R.N.) licensure in state of practice. 
  • Bachelor degree preferred
  • BLS required.



  • Minimum 3 years clinical.
  • Must have good communication skills.
  • Must have strong organization skills and be able to work independently.
  • Previous geriatric/rehab nursing experience preferred, or will orient to point of competency



  • Preferred: knowledge of quality improvement processes with an emphasis on clinical documentation standards, auditing, and reimbursement process.



  • Work requires normal amounts of physical effort up to 66% of the time.    
  • Ability to lift objects weighing 10 lbs. or less.          
  • Work is performed under normal working conditions with adequate lighting and ventilation.
  • Duties require the ability to concentrate and pay close attention to detail (more than 66% of work time).
