PeaceHealth is seeking an experienced RN for a Geriatric Fractures Program Coordinator role at  John Medical Center in Longview, WA.  This is a part-time, 0.6 FTE, working 24 hours per week, with the flexibility of working either four 6-hour shifts or three 8-hour shifts.

JOB SUMMARY

Working in tandem with the Joint Replacement Program Coordinator this role is responsible for overall implementation and operations of the hospital’s fracture program. The coordinator navigates patients and families through the Geriatric Fracture Program (GFP) process. The coordinator develops implements, evaluates, and revises the plan of care based on physical, psychosocial, spiritual and educational needs of the patient and family. The coordinator monitors the environment and initiates interventions to provide safe patient care. The GFP coordinator co-manages the day to day operations of the center and is responsible for following patients up to 90 days after surgery at participating facilities including rehabilitation hospitals, assisted living, and skilled nursing facilities. This includes on-site visits, assisting with wound care, therapy and family/social issues.  This role works with several key stakeholders to coordinate this work; Rehab, Surgery, Marketing, Nursing, Pharmacy and surgeons, etc.

 

 

ESSENTIAL FUNCTIONS

  1. Perform comprehensive physical assessment of patient’s clinical condition and psychosocial needs inclusive of family needs. Subsequently develops plan of care based on findings and multidisciplinary provider team input. Assesses patient’s physical and emotional response to treatment, assists patient and family in coping with patient’s condition then modifies care plan appropriately if necessary.  Evaluates the effectiveness of interventions, patient outcomes, and revises planned care accordingly.
  2. Implements plan of care safely and efficiently. Evaluates the effectiveness of interventions, patient outcomes, and revises planned care accordingly.
  3. Collaborates with and assists staff, physicians and other health team personnel in the delivery of coordinator patient care.
  4. Develops and maintains relationships with rehabilitation facilities and key primary care providers. 
  5. Serves as a liaison with team members, PeaceHealth departments and external stakeholders to ensure the continuum of care for patients, families and development of GFP.
  6. Coordinates and leads regularly scheduled GFP PIT meetings effectively with hospital administrators, the unit, medical director, other physicians and key staff, and assists with facilitating community education programs.
  7. Maintains patient records and charts to ensure accurate communication and continuity of patient’s physical and emotional response to treatment.
  8. Responsible for updating orders, pathways, databases, and other documentation related to the Geriatric Fracture Program.  Responsible for conducting data analysis, interpreting data for analysis of clinical results, reporting out to key stakeholders on trends, program effectiveness and recommending improvements either in the program or clinical care methodologies as appropriate.
  9. Participates in orientation and facilities the learning experience of new caregivers regarding the Geriatric Fracture Program.
  10. Advances personal growth through participation in educational programs, current literature, in-services and workshops.
  11. Performs other duties as assigned.

 

 

QUALIFICATIONS

EDUCATION:

  • Graduate of an accredited school of nursing required.
  • Bachelor’s degree; or equivalent knowledge and skills obtained through a combination of education, training and experience.
  • RN BSN preferred.

 

EXPERIENCE/TRAINING:

  • Minimum of three years of clinical experience is required.
  • Minimum of five years of geriatric program experience preferred.
  • Prior experience working with multidisciplinary team is preferred.
  • Prior leadership experience, including managing resources either directly or within the implementation of a new initiative is preferred.

 

LICENSE/CERTIFICATION:

  • Current WA State RN license is required.
  • Current BLS (Basic Life Support) certification required at date of hire.

 

OTHER SKILLS:

  • The ability to demonstrate a courteous, caring and understanding attitude toward patients, families, co-workers, visitors and physicians is required.
  • Strong data analysis skills, requiring interpretation of clinical results to develop program improvements projects is required.
  • Detail-oriented, excellent organizational skills, ability to multitask, and capacity for follow through is required.
  • Experience in achieving results and working with process management.
  • Excellent communication skills; written, verbal and excellent presentation skills are required.
  • Ability to keep team focused on goals/timelines is required.
  • Excellent customer service skills are required.

 

LCR PHYSICAL/COGNITIVE REQUIREMENTS:

  • Work requires moderate physical exertion from 34 to 65% of the time.      
  • Ability to lift objects weighing 30 lbs. or less.          
  • Work is performed under normal working conditions with adequate lighting and ventilation.        
  • Job duties frequently require intense concentration and attention to detail (34-65% of work time).
