Manager Heart Failure Program - Nursing Administration

PeaceHealth is seeking a Manager Heart Failure Program - Nursing Administration for a 1.0 full time equivalent Exempt position. This position will be working Day shift with shift durations of 8 Hours.

JOB SUMMARY

Responsible for operational performance, quality, service environment and regulatory compliance of the heart failure program.  Works collaboratively with physician leadership, clinical management, education department, and clinical staff to ensure overall integration throughout the organization. In addition, this role is responsible for ensuring that system and processes are developed to provide measurable improvement in clinical outcomes through the application of best practices and current technologies.

 

ESSENTIAL FUNCTIONS

1. Accountable for the overall management of the heart failure program. Facilitates the clinical care of heart failure patients to assure that all patients receive comprehensive and timely treatment/interventions that are consistent with defined clinical practice guidelines. Collaborates with Critical Care Director, internal and external providers (home health agencies/skilled nursing facilities) and leads multidisciplinary team to develop patient-specific heart failure protocols to prevent unnecessary admission/readmission. Maintains in-depth knowledge of ACC/AHA/HFSA guidelines and expertise in heart failure pathophysiology and patient management.

 

2. Compiles, analyzes and interprets key data of heart failure patients. Facilitates regular multidisciplinary team meetings, including heart failure steering committee and heart failure discharge readiness workgroup. Provides timely feedback and guides progress toward goals. Compares outcomes against program goals.  Participates in protocol design for data collection, feedback and analysis.  Utilizes the concepts of Quality Improvement when planning and implementing process changes and follows process through to resolution. Conducts quarterly reporting of program effectiveness in order to ensure long-term sustainability. Documents accurately and thoroughly. Participates in other committees, meetings, patient rounds/care conferences and workgroups as assigned.

 

3. Provide formal and informal education to patients, patient family members, team members and the community and acts as a resource for team members, medical staff and the community. Assure that standards for patient education and educational materials are followed. Maintain knowledge of available community resources for heart failure patients.

 

4. Facilitate transition across inpatient and outpatient care settings; assists with development and implementation of appropriate care across the continuum of care for heart failure patients. Acts as a resource and consultant on issues of standards of care for heart failure patients. Assures adherence to the established plan of care. Ensures coordination of referrals to appropriate level of care and resource,

 

5. Provide human resource management of staff and/or volunteers including hiring, coaching, counseling, mentoring, evaluating, competency achievement, training. recognition and disciplinary action when indicated. Provide direct leadership by ensuring efficient and effective operation of assigned program in compliance with hospital standards and federal, state and local laws. Develop, implement and apply department policies and procedures. Ensure safety and risk management guidelines are implemented and practiced. Provide financial/resource management by assisting with budget development, ensuring productivity standards are met and monitoring expense activities to meet budgetary requirements.

 

6. Performs other duties as assigned.

 

QUALIFICATIONS

 

EDUCATION:

  • Graduate of accredited nursing program. BSN required.
  • MSN preferred.

 

EXPERIENCE/TRAINING:

  • Minimum of 5 years of related experience, including experience directly related to cardiology.
  • Minimum of 2 years of leadership experience required.
  • Demonstrate competence in area of expertise.
  • BLS
  • ACLS
  • Experience in Heart Failure

 

LICENSE/CERTIFICATION:

  • Valid Washington RN license.
  • Certified Heart Failure Nurse (CHFN) required at time of hire.

 

OTHER SKILLS:  

  • Ability to deliver financial results for areas of accountability.
  • Experience should include process improvement, systems redesign, quality measurement, data analysis and reporting, data integrity, team-based improvement, improvement training, and team facilitation.
  • Demonstrated leadership abilities with skills to develop and direct a multidisciplinary team
  • Demonstrated ability to prioritize and manage multiple tasks at one time
  • Demonstrated ability to promote positive relationships and a customer-focused work environment
  • Demonstrated ability to work effectively within a self-directed environment
  • Proficient computer skills including Microsoft Excel, Word and PowerPoint
  • Knowledge of current National and State Heart Failure guidelines
