System Director - Care Management

PeaceHealth is seeking a System Director - Care Management for a 1.0 full time equivalent Exempt position. This position will be working Day shift with shift durations of Variable.

JOB SUMMARY:

Responsible for the executive leadership, direction and system operations of the multi-hospital team of directors and managers in Care Management. Establishes strategic and results oriented tactics, goals, and objectives using a collaborative team approach. Develops system-wide long and short-term business strategies/operational plans to align with organizational strategy and oversees implementation. Responsible and accountable for care coordination practice models that support quality patient care that is safe, timely, effective, equitable, and patient-centered. Ensures systems and processes are in place to support regulatory and accreditation compliance with state, federal, and related accrediting bodies for all programs within the scope of assigned departments.  Ensures necessary human and supply resources are available for delivery of expected services. Responsible for incorporating nationally recognized Scope of Services and Standards of Practice for care management into the program and validates knowledge, competency and skills

 

ESSENTIAL FUNCTIONS:

  1. Understands and promotes organizational mission, values and behavioral expectations.
  2. Provides leadership and direction for the department; creating and communicating a vision that motivates them to pursue high standards of excellence, including validation of competency and skills, critical thinking and decision making. 
  3. Communicates with varying levels of staff, managers, executives, and physicians. Leads education regarding care management as well as collaborating on joint projects.
  4. Collaborates with organizational leaders to ensure ongoing survey readiness and identifies opportunities for improvement within department. Contributes to organizational preparedness through leadership, staff engagement and committee participation.
  5. Ensures policies and procedures related to Care Management are current and relevant and updated to reflect regulatory changes.
  6. Works to understand the importance of the Quadruple Aim (population health, cost of care, patient experience, and care team well-being.) Works closely with care management leaders to ensure network specific programs are in place to focus on care team well-being in order to optimize performance and patient outcomes.
  7. Uses data and technology necessary to review, interpret, compile, and provide reports.
  8. Works with care management leaders to ensure accountability standards/metrics are being measured and reported. Works closely and coaches’ leaders who demonstrate opportunities for improvement.
  9. Develops, implements, and maintains ongoing performance improvement and quality improvement initiatives applying Lean Management Philosophy, Systems, and Tools. Applies this philosophy to develop accountability standards for the system.

 

 

QUALIFICATIONS

EDUCATION:          

  • Master’s degree in nursing is required.  

 

EXPERIENCE/TRAINING: 

  • Minimum of 7 years of related healthcare management required.

 

LICENSE/CERTIFICATION

  • Current RN licensure for profession from the state in which caregiver is practicing required.
  • ACMA or CCM Certification within 1 year of hire

 

KNOWLEDGE/SKILL/ABILITIES 

  • Ability to deliver financial results for areas of accountability. 
  • Strong communication skills, both verbal and written.
  • Strong interpersonal skills.
  • Ability to work in an interdisciplinary environment with individuals at all levels of the organization.
  • Proficient computer skills including MS Office applications.

 

PHYSICAL & COGNITIVE REQUIREMENTS:

  • Work requires fairly light physical exertion from up to 65% of the time.         
  • Ability to lift objects weighing 30 lbs. or less.          
  • Work is performed under normal working conditions with adequate lighting and ventilation.     
  • Job duties frequently require intense concentration and attention to detail (up to 65% of work time).
